Case Management

Most central and foundational to recovery is Case Management - the monitoring and coordination of all facets of client care. From the time of the initial assessment, our seasoned Case Management Advocates plan, organize, and execute every aspect of our individualized client recovery plan in order to accomplish predetermined recovery goals for our clients and their loved ones, including:

To start, TAG architects the recovery plan from the initial evaluations and assessments under the direction of our elite clinical team. We then implement support systems specific to the client’s mental, physical and spiritual needs ensuring that all markers and timelines are met and adhered. TAG collaborates with all clinical, medical and ancillary treating partners and directs the client’s recovery services, ensuring client accountability. 

The Advocates manage the complete 360 picture of client wellness until they reach full recovery. Our goal is complete client autonomy, achieved and maintained through the continued participation in the daily practices provided in the plan.

 

Transportation

Transportation is a pivotal and critical part of the intervention process. Our seasoned team understands the difficulties that can arise during this delicate transition. Transportation Advocates assist clients in the shift from identified patient to the individual choosing to walk through the doors of the treatment program.

We have set travel protocols in place that adhere to strict safety guidelines. A medical screening is thoroughly reviewed to determine the best mode and time of transit. We handle all details from admission requirements, financial arrangements and insurance coordination, to the packing and travel booking. We communicate with all necessary members of the treating team, and family when applicable, throughout the transport to ensure a seamless admission process. 

Transportation services are also available for clients to and from work, appointments, family sessions and any event that the team deems that added support would be beneficial.

 

Companion

TAG understands that the steps required to transition to a life of recovery can be extremely challenging for even the most accomplished of individuals. Being new to recovery is a critical time when clients can feel exposed and may be vulnerable to relapse. To solidify the transition to a life of full recovery, Companions work as essential partners and are available to stay with clients 24/7. Companions are not babysitters; their goal is to move each client forward to a life of self-sufficiency and success on their own terms. 

Companions meet strict guidelines: each is screened and cleared as well as CPR, First Aid, and Narcan trained. Companions are also trained in the art of crisis intervention and de-escalation with client care and safety at the forefront of all they do. 

The TAG Companion directs the individualized Recovery Plan for the client, provides support around the clock to monitor safety and assist with the development of daily living skills. Companions also provide support for the re-entry into existing positions, career, employment, school and community or assist in the development of new ones. Our goal is the complete autonomy and wellness of each client.

Intervention

An Intervention is a carefully planned process where family and friends confront their loved one about their addiction.

The Advocacy Group establishes client needs by assessing the individual and the family requesting the intervention. TAG begins the intervention process by meeting with the family, and friends and colleagues if appropriate, and develops a bespoke pre-intervention plan of action. 

As part of this process, The Advocacy Group offers family empowerment, education, and support to initiate the client’s loved one into their own recovery. TAG designs and discusses how to enforce boundaries, when necessary, with all Intervention stakeholders - family, friends, and colleagues. We strive to infuse hope and solutions where it currently feels like there are none.

The intervention itself is a time of opportunity for the family to speak truth to their loved one and confront them about their addiction. It is a time for the client to hear the boundaries and consequences their inner circle has put into place and to make the conscious decision to choose recovery and seek treatment. 

TAG also recommends treatment options and assists in treatment placement based on client needs.

 

Family Empowerment

The Advocacy Group Family Empowerment service is a critical and essential component of recovery. Family members of those struggling with addiction and mental illness have been subjected to pain, frustration, powerlessness and life disruption long before seeking help. Recovery is possible for all, especially for the family.

TAG will design a family specific plan grounded in empowerment, education, awareness, and support services that are infused with hope and healing. 

When one person is facing addiction, the entire family system breaks down. All members of the family must commit to a healing process and do the work required to restore the family to wholeness. 

We invite the family to release themselves from all that they cannot control and take the power back where they can.

 

Coaching

The Advocacy Group offers bespoke, one-on-one Coaching services to clients in need of guidance and support when making the transition from treatment to a complete life of independence. 

Coaching is available on an hourly basis to help clients fully attune one’s life to sobriety. Our coaches transport and guide clients in all aspects of their life including: 

  • Setting up a daily routine

  • Transitioning to living alone 

  • Integrating into a recovery community 

  • Building healthy habits, including nutrition, meditation, and fitness

  • Family engagement and communication 

  • Reentering one’s profession

  • Seeking education and opportunities for a new profession

  • Wellness practices to cement recovery 

  • Healthcare appointments 

  • Seeking and sustaining educational opportunities

We utilize our onboarding procedures, assessments, and Recovery Plan to drive the coaching process and best advocate for our clients. We empower our clients to take the steps to complete autonomy.

Court Advocacy

The Advocacy Group is acutely aware that many individuals come into recovery with unfinished business in the legal realm. 

Our Court Advocates are trained court liaisons providing emotional support and transportation to all proceedings. Our role is not to give legal advice, but to instead provide emotional support and help navigate the judicial system. 

Court Advocates possess the expertise in navigating the stressors that are involved in facing the justice system. TAG Liaisons know that each situation is unique and deserves to be treated as such. Our Court Advocacy plan honors the seriousness of the task at hand as well as reinforcing the freedom inherent in a life of recovery. 

The Advocacy Group believes that there is no problem without an avenue for solution. There is nothing our clients will have to face alone.
